He's mad. I realize that's not a technical term, but really, he's mad.
He thinks he's now (bravely) imposing a tariff on imports of fentanyl. "In order to make it more costly to distribute and buy." His customs agents, when they catch people bringing fentanyl across the border, will "penalize them"—by making them pay tariffs.
He's not just mad. He's also stupid.
He's also cementing his party's control over the American electoral system, without significant opposition. At this rate only a coup will remove them from power.
Trump is not necessarily stupid. He might deliberately be creating waves on the stockmarket so that he can surf them as an inside trader. The general trend becomes irrelevant to him.

The only people this will bbn impact is cancer and similar patients with severe pain for which this is the only control.
The only fentanyl imports that can be tarriffed, are legal pharmaceutical imports.
